Income-based Jobseeker’s Allowance
Income based Employment and Support Allowance
Pension Credit Guarantee Credit
Tax Credits and entitled, or named on a valid NHS tax credit exemption certificate
On a low Income and named on a valid HC2 (full help) certificate
On a low Income and named on a valid HC3 (partial help) certificate
NHS complex lens voucher
The optometrist will issue the voucher for your spectacles after your eye examination
